Insurance premiums are not subject to Indiana sales tax, and Indiana does not tax services generally. You only need a Registered Retail Merchant Certificate (RRMC) if your agency sells tangible personal property at retail. Businesses also owe 7% use tax on taxable items bought without tax paid (e.g. out-of-state office supplies). Register through INBiz using the BT-1 business tax application.

1 stopIndianapolis — No General Business or Occupational License
Indianapolis has no general business or occupational license. The Department of Business and Neighborhood Services licenses only the specific business types set by ordinance — among them adult entertainment, alarm companies, amusement locations, carriage and coach operators, commercial parking facilities, dance halls, escorts, fire prevention businesses, hotels and motels, kennels/pet stores/stables, massage businesses, pawn brokers, pedal cabs, scrap and salvage companies, short-term rentals, taxis, ticket brokers, tobacco specialty bars, towing, transient merchants, valet zones, vendor carts, and waste haulers. Insurance agencies are not among them: insurance producers and agencies are licensed at the state level by the Indiana Department of Insurance. A home-based agency must comply with residential zoning use rules, but Indianapolis has no separate 'Home Occupation Permit'.
