1. Alaska Board of Barbers and Hairdressers — Tattoo Shop and Practitioner Licenses

   Tattoo shops in Alaska are licensed by the Board of Barbers and Hairdressers within the Division of Corporations, Business and Professional Licensing (CBPL) — not by the Department of Environmental Conservation. The owner files a Shop Owner License application (form 08-4191), which covers shops performing tattooing, permanent cosmetic coloring, body piercing, hairdressing, barbering, esthetics, or manicuring, and must include a satisfactory sanitary health inspection certificate. Individual artists are separately licensed by the same board (form 08-4464) after passing a Prov examination and verifying CPR and bloodborne pathogens training. DEC sets the sanitation regulations for body art shops but charges no fee. The sanitary inspection is DEC’s Certificate of Sanitary Standards under 18 AAC 23.310, which carries no DEC fee and is renewed annually; DEC sends proof directly to CBPL.

2. City of Fairbanks — City Business License

   Every business operating inside Fairbanks city limits needs a City Business License from the City Clerk’s Office (800 Cushman Street, 907-459-6702), in addition to the Alaska state business license.
