3 stopsIndiana Secretary of State — Business Entity Registration (INBiz)
Indiana issues no state fitness-studio license, so registration is the ordinary business-entity filing. File Articles of Organization (LLC) or Articles of Incorporation (corporation) with the Secretary of State through INBiz to limit personal liability, and name a registered agent with an Indiana address. After formation, register with the Department of Revenue for any tax types you need and obtain local building, zoning and occupancy approvals. Every entity must then file a Business Entity Report every two years or be administratively dissolved.
Official government source$95.00 domestic LLC Articles of Organization filed online through INBiz (plus a card processing fee of at least $1, capped at 2.15%); Business Entity Report $32.00 online or $50.00 by paper, due every two yearsSame day to 1 week online via INBiz

Indiana Department of Revenue — Registered Retail Merchant Certificate (Sales Tax)
Indiana taxes tangible personal property, not services, so membership dues and class fees are generally not subject to sales tax. If your studio sells any tangible goods — merchandise, apparel, bottled drinks, protein shakes or supplements — you must hold a Registered Retail Merchant Certificate (RRMC) and collect Indiana's flat 7% state sales tax (there is no local sales tax). Register through INBiz using the BT-1 business tax application.

Indiana — No State Personal Trainer or Group Fitness Instructor License Required
Indiana does not license personal trainers or group fitness instructors — neither appears on the Indiana Professional Licensing Agency's list of regulated professions. (Athletic Trainer is a separate licensed healthcare profession and the title may not be used by fitness staff.) Certification is therefore a business and insurance requirement rather than a legal one: most studios require staff to hold a credential from an accredited body such as NASM, ACE, ACSM, ISSA or NSCA, plus current CPR/AED certification, and liability insurers commonly require the same.
Official government sourceNo state license or fee. Voluntary certification is priced by each certifying body; CPR/AED certification is separate and inexpensiveN/A for the state; 4–12 weeks of self-study if pursuing certification

Indianapolis — Fire and Building Inspection for an Assembly Occupancy
A fitness studio open to the public must meet fire and building code for occupant load, exits, extinguishers, sprinklers, and egress paths. In Indianapolis these inspections are carried out by the Department of Business and Neighborhood Services, which inspects premises where municipally licensed activities are carried out and inspects construction work under its building permits; the Indianapolis Fire Department supports fire code enforcement. Any buildout or change of use also requires DBNS permits and their own inspections before the space is used.
Official government sourceDBNS fee schedule effective January 5, 2026: fire inspection of premises used for municipally licensed activities $84 and building inspection $84, each charged for the initial inspection and the annual reinspection; general construction inspection not otherwise specified $154; reinspection $175. Accelerated inspection options are offered for a fee — for a Class 1 structure, $300 for two hours for a next-day inspection or $750 for two hours for a same-day inspection.
